Yep, weight too much on your heels causing you to shift to your toes through the shot to keep balance. To be fair you do this pretty well, but it'll make you inconsistent.
apart from that and a bend of the left elbow at the top it's very solid looking. I'd move your weight to the balls of your feet a little and concentrate on making what feels like a 3/4 swing. You'll hit it just as far but better.
