The Northern Irishman smashed a lady's phone with his tee shot down the first hole en route to a quadruple-bogey eight and then duffed a chip on the third to play the first three holes in 5-over par.
McIlroy bounced back with birdies at 7 and 9 but then handed them both back on the 16th.
