Fleetwood endured his latest heartbreak on the PGA Tour at the FedEx St. Jude Championship in Memphis.
The Englishman, 34, held a two-shot lead with three holes to play but once again appeared to crumble like a pack of cards down the stretch.
It resurrected memories of what transpired at the Travelers Championship in June when US Ryder Cup captain Keegan Bradley pipped Fleetwood.
Fleetwood now has 29 top-five finishes on the PGA Tour, including six runner-up finishes, from 162 starts.
"The problem for Tommy is that he's not confident, he's not, he's [not] got this winning killer mentality yet," Pepperell told the latest edition of the Chipping Forecast.
"He can grow and build but he's not playing in events where I think it can, it can, he can just get over the line by someone else not doing what Justin Rose has just done.
"He's playing in these tournaments where you're having to beat Scottie Scheffler and if you're not trying to beat Scottie Scheffler you're trying to beat these world-class, immensely confident killers and that just makes it so much harder for Tommy.
"And so I just wonder if there's a, there's a place in his mind where next year he tries to add in a few of the lower PGA Tour events and go and win by four or five or six, dominate one the way Cam Young did because he will go from strength to strength now Cameron Young.
"We see it so regularly. I just, yeah that would be my only wonder if Tommy does that scheduling wise next year."
"I need to be very careful what I say"
Rory McIlroy's caddie Harry Diamond came in for heavy criticism at times over the past decade as the Northern Irishman attempted to end his major drought.
McIlroy, of course, had a few words for his pal's critics after the pair finally got over the line at the 2025 Masters.
He was reduced to tears as he explained just how crucial Diamond has been to his success.
Pepperell previously speculated on the subject, hinting that McIlroy may have got over the line at the 2024 U.S. Open at Pinehurst No.2 had a 'stronger' figure been carrying his sticks.
Fleetwood has been working with his looper, Ian 'Fino' Finnis since 2016. The pair are close pals and the looper was even his best man when the golfer married Clare Craig.
"I drew on his confidence down the stretch and that is a big, big thing."
Pepperell also touched on the body language of Justin Rose's caddie Mark 'Fooch' Fulcher.
Rose went on to win the FedEx St. Jude Championship, prevailing in an electric playoff over US Open champion JJ Spaun.
"Fooch looks a killer as well," Pepperell said. He added: "Look Fino, I'm sure, does add way more to Tommy than we could all imagine.
"He's close friends, always has been but in the in the interest of winning a tournament and getting over the line are there people out there that could help Tommy do that?
"I think there probably are and I would have said that about Harry Diamond and Rory a year ago and I, and I still think there's some truth in that to be quite honest with you but I'm not saying that you know Fino should lose his job. I don't want that to go out there or anything like this."
Fleetwood is back in action this week at the BMW Championship at Caves Valley Golf Club in Maryland.
As we have come to expect from Fleetwood, he refused to sulk after his latest near miss.
"I would rather be there and fail than not be there at all," he said.
